Follow the procedure below to check the status of your annual evaluation forms
Visit your NSS dashboard @ Portal.nss.gov.gh
Select sign in as personnel
Enter your email address and password
Scroll down to annual submission status
You will see Change Request Approved.
From there it means your Annual Evaluation has been Submitted or not, if it has been submitted you will see Annual Evaluation Submitted.
